Read all the questions on your screen then scroll down until the next question is at the top of the window and read another full screen of questions.
This method saves time. If your answer to a question would be "No", "Never" or "None" Do NOT answer, please Skip to the next question!
Only mark answers to those conditions or symptoms which you have.
The Symptom Survey is presented in three parts and advances automatically from part to part.
You only have to finish one part at a time, and you may take breaks between parts.

Below is the an explanation of terms used in the Symptom Survey:
"No" means No.
"Never" is once a year or less.
"Rarely" is once a month or less.
"Often" is more than once a month.
"Usually" is once a week or twice a week.
"Always" is several times a week or more.
"Mild" means occasionally noticeable but not too bothersome.
"Moderate" means usually noticeable and often bothersome.
"Severe" means usually or always noticeable and quite bothersome.
"Yes" means Yes.
If your answer to any question would be
"None",
"Never"; or
"No."
You do not need to answer, just skip to the next question!
You need only respond when your answers are one of the other choices.
The Symptom Survey was designed to assist those who are seeking
natural nutritional solutions to their health problems.
To save time in satisfying those who are merely curious,
