Below is the an explanation of terms used in the Symptom Evaluator:

"Mild" symptoms occur only once or twice a year may be bothersome or not.
"Moderate" symptoms occur several times a year and may be bothersome.
"Severe" symptoms occur most of the time and are quite bothersome.
